Aetna’s HMO Medicare Premier Plan has a monthly premium of $0.  For primary care visits there is $10 copay when the primary care physician is notified.  For specialist visits there is $30 copay and members receive one annual physical exam and one annual eye exam.  There is $100 copay for the first 5 days for impatient hospital care and $150 copay per outpatient visit.

Aetna’s PPO Medicare Value Plan has a monthly premium of $35.  For primary care visits there is $10 copay when the primary care physician is notified.  For specialist visits there is $35 copay and members receive one annual physical exam and one annual eye exam.  There is $150 copay for the first 5 days for impatient hospital care and $150 copay per outpatient visit.

Aetna’s PPO Medicare Standard Plan has a monthly premium of $60.  For primary care visits there is $10 copay when the primary care physician is notified.  For specialist visits there is $35 copay and members receive one annual physical exam and one annual eye exam.  There is $150 copay for the first 7 days for impatient hospital care and $100 copay per outpatient visit.
