For specialist visits there is $40 copay and members receive one annual physical exam. There is $225 copay per days 1-7 for inpatient hospital care and $0-250 copay per outpatient visit. HumanaChoice R5826-065 Regional PPO has a monthly premium of $0. For primary care visits there is $10 copay when the primary care physician is notified. For specialist visits there is $30 copay and members receive one annual physical exam and one annual eye exam. There is $550 copay per stay for inpatient hospital care and $0-95 copay per outpatient visit. Pharmacy is not included within this plan.
