HumanaChoice R5826-068 Regional PPO has a monthly premium of $0.  For primary care visits there is $10 copay when the primary care physician is notified.  For specialist visits there is $30 copay and members receive one annual physical exam and one annual eye exam.  There is $550 copay per stay for inpatient hospital care and $0-$95 copay or 20% copay per outpatient visit.
