For specialist visits there is $45 copay and members receive one annual physical exam and eye exam. There is $150 copay per days 1-5 for inpatient hospital care and $150 copay per outpatient visit. Aetna’s PPO Medicare Value Plan has a monthly premium of $50. For primary care visits there is $15 copay when the primary care physician is notified. For specialist visits there is $45 copay and members receive one annual physical exam and eye exam. There is $100 copay per days 1-5 for inpatient hospital care and $100 copay per outpatient visit. For specialist visits there is $40 copay and members receive one annual physical exam and eye exam. There is $750 copay per stay for inpatient hospital care and $100 per outpatient visit.
