HumanaChoice H5826-079 has a monthly premium of $56 in Alexandria, VA. This Medicare Plan has a $15 co-pay for primary care office visits and a $40 co-pay for specialty office visits. Hospital Inpatients (those who require an over-night stay at the hospital) will have $225 co-pay per day for the first seven days. Day 8-90 will have a $0 co-pay with this Plan. Hospital outpatients (those who require a hospital stay that is not over-night or who require medical equipment or service) will have a $0-$250 co-pay per visit, depending on the cost of the visit.
