Guardian Healthcare Medicare
Guardian Healthcare is a Medicare Advantage Organization which offers medicare plans throughout South Carolina. They offer a variety of four medicare plans- the Tribute Plan, the Tribute Enhanced Plan, the Tribute Pride Plan, and the Tribute Select Plan. Those who are eligible for these plans are entitled to Medicare Part A and who are enrolled in Medicare Part B.Tribute Plan
There is a $0 monthly plan premium in addition to the beneficiary's Medicare Part B premium along with a $5,000 out-of-pocket limit. The beneficiary is able to go to any doctor, specialist, or hospital that accepts the plan's payment. There is a $200 co-pay per day from days 1-5 for Medicare-covered hospitals and $0 co-pay from days 6-90. For mental health care, there is a $300 co-pay per day from days 1-5 and $0 co-pay per day from days 6-90. For a skilled nursing facility, days 1-10 are covered while days 1-20 a beneficiary must pay a $50 co-pay per day and a $124 co-pay per day from days 21-100. There is a $10 co-pay for each primary care doctor visit and a $35 co-pay for each specialist visit.
Tribute Enhanced Plan
There is a $28.50 monthly plan premium in addition to the beneficiary's Medicare Part B premium along with a $4,000 out-of-pocket limit. The beneficiary is able to go to any doctor, specialist, or hospital that accepts the plan's payment. There is a $100 co-pay per day from days 1-7 for Medicare-covered hospitals and $0 co-pay from days 8-90. For mental health care, there is a $300 co-pay per day from days 1-5 and $0 co-pay per day from days 6-90. For a skilled nursing facility, days 1-10 are covered while days 1-20 a beneficiary must pay a $50 co-pay per day and a $124 co-pay per day from days 21-100. There is a $10 co-pay for each primary care doctor visit and a $30 co-pay for each specialist visit.
Tribute Pride Plan
There is a $0 monthly plan premium in addition to the beneficiary's Medicare Part B premium along with a $3,400 out-of-pocket limit. The beneficiary is able to go to any doctor, specialist, or hospital that accepts the plan's payment. There is a $75 co-pay per day from days 1-3 for Medicare-covered hospitals and $0 co-pay from days 4-90. For mental health care, there is a $75 co-pay per day from days 1-3 and $0 co-pay per day from days 6-90. For a skilled nursing facility, there is a $0 co-pay. There is a $5 co-pay for each primary care doctor visit and a $10 co-pay for each specialist visit.
Tribute Select (PPO)
Tribute's Select Plan is available only for those who live in Greenville, South Carolina. There is a $0 monthly plan premium in addition to the beneficiary's Medicare Part B premium along with a $3,400 out-of-pocket limit. The beneficiary is able to go to any doctor, specialist, or hospital that accepts the plan's payment. There is a $0 co-pay per day from days 1-2 for Medicare-covered hospital stays, $100 co-pay per day from days 3-6, and $0 co-pay from days 7-90. For mental health care, there is a $200 co-pay per day from days 1-5 and $0 co-pay per day from days 6-90. For a skilled nursing facility, days 1-10 are covered while days 1-20 a beneficiary must pay a $50 co-pay per day and a $124 co-pay per day from days 21-100. There is a $0 co-pay for each primary care doctor visit and a $20 co-pay for each specialist visit.
