HealthPartners Medicare
HealthPartners Medicare plan is offered in Minnesota and Wisconsin. To be eligible, beneficiaries must be 65 years or older and must be enrolled in either Medicare Part A or Part B.Freedom Plan I
Freedom Plan I includes a $59 monthly premium. With this plan, members pay a 20% copay during doctor visits and a $300 copay for a hospital stay. There is no dental coverage, Part D options, or travel coverage
Freedom Plan II
Freedom Plan II includes a $90 monthly premium. With this plan, members pay a $15 copay during doctor visits and and a $150 copay for a hospital stay. Dental insurance is covered for one cleaning, one exam, and one x-ray annually. Standard part D plan is included. For traveling, 9 months within the U.S. and worldwide emergency care is covered.
Freedom Plan III
Freedom Plan III includes a $130 monthly premium. With this plan, members pay a $0 copay during doctor visits and $0 copay for a hospital stay. Dental insurance is covered for one cleaning, one exam, and one x-ray annually. Three part D plans are included. For travelling, 9 months within the U.S. and worldwide emergency care is covered.
