Medicare Premier Plan HMO has a monthly premium of $100.  For primary care visits there is $20 copay when the primary care physician is notified.  For specialist visits there is $40 copay and members receive one annual physical exam and one annual eye exam.  There is $750 copay per stay for inpatient hospital care and $100 of copay per outpatient visit.

Humana Medicare began marketing health insurance products in 1984.  The monthly premiums for Humana’s plans range from $0-$76. 

Medicare Supplement Plan F offers basic copay coverage as well as Part A hospital deductible, skilled nursing facility copays, Part B deductible and emergency international coverage.

Mutual of Omaha’s Supplement Plan F has a monthly premium of $136.13 and is accepted by all doctors who accept Medicare.   Aetna’s Supplement Plan F has a monthly premium of $172.95 and is also accepted by doctors who accept Medicare.
